What causes memory loss in teens?

I have an atrocious memory. I can’t remember if I’ve eaten, I can’t remember how to tell if I’m full or hungry, I can’t remember if I’ve slept, I can’t remember… well anything really. I can remember the English language, that’s pretty much it. I’ve never had a good memory, but it’s gotten steadily worse over the years (I’ve verified this).
Just to note, I don’t take drugs of any kind (caffeine and alcohol included). I read and do other mentally stimulating activities constantly. I have suffered a great amount of head injuries (self inflicted) , but I don’t have any brain damage as far as the doctors can tell.
I have depression (at least, I think I still do. I can’t remember the last time I had depressive feelings), and my family has a few members with mental illness, though nothing that could terminate memory to this extent as far as I know. I don’t remember any of my childhood, and although abuse is a possible explanation, I have no proof.
To reiterate, what causes memory loss in teenagers (ages 14-17)? Any and all help is appreciated. I really want to be able to concentrate and remember things again.
